At Care Safety Reports, we believe that transparency is the first line of defense in elder care. Choosing a nursing home is often a decision made in crisis, and families rarely have the time to comb through thousands of government PDFs to find the truth. We exist to solve one problem: Making federal safety data accessible, readable, and actionable.
Where Our Data Comes From
We are an independent data transparency initiative. We do not inspect facilities ourselves. Instead, we aggregate publicly available data from the Centers for Medicare & Medicaid Services (CMS), specifically the "Provider Data Catalog."

Our Methodology
Unlike other directories that prioritize "curb appeal" or marketing photos, Care Safety Reports prioritizes safety metrics. We highlight critical deficiencies—such as medication errors, fall risks, and staffing shortages—so families can ask the right questions before signing a contract.
